PRIVACY STATEMENT

Logan Central Respite Centre needs to collect your personal information that may include your name, birth date, current address, phone numbers, next of kin, Doctors name and any medical issues you may have. All your information is stored in a confidential locked file in the Coordinators office.

LCRC understands you may not want to provide this information to us. If you decline or refuse LCRC may not be able to provide you with the comprehensive service you require. LCRC clients are able to access their personal information and to make any changes to their information at any time by speaking to the Coordinators.

 

COLLECTION STATEMENT

We collect personal information, directly from you or your carer via phone, or referral assessment forms (ONI) from other organisations once you have given LCRC permission to do so. Your personal information is used to identify you, provide, manage and administer your services and help us deliver appropriate services to you.

We share your information only with your permission to Government funding bodies or other services to enable us to improve our service. Your privacy is always protected. All clients are given an identification code for security of their personal information. With your permission your personal information may be shared with other services who may be able to offer appropriate additional services to you.

In the case of an emergency your health details would be provided to the Ambulance service, with your permission.

LCRC clients can be assured their personal details are never shared with Direct Mail or Telemarketers.

 

COMPLAINTS SYSTEM

LCRC has a complaints system in place in which any client/carer may lodge any concern or complaint they may have regarding their privacy or any other issues that may arise during the time you access LCRC services.

A Complaints leaflet and a Privacy Leaflet are included in our Information Pack. They explain how you can make a complaint and how your complaint will be dealt with.
